A 64 year-old man presented with brief spells of difficulty expressing his words and right arm weakness.

Show the Critical Stenosis and "String" Sign of the Internal Carotid

Internal Carotid Artery Stenosis: Angiogram of the Left Common Carotid Artery (CCA). Note there is a critical stenosis of the left internal carotid artery (ICA) just distal to the carotid bifurcation. In this patient, the carotid ultrasound suggested a complete occlusion of the internal carotid artery. This differentiation is important because patients with high grade stenosis benefit from carotid endarterectomy compared to conventional medical treatment, whereas patients with complete occlusions are not surgical candidates. ECA = external carotid artery.
