Posterior Cerebral Artery Branch Infarction - Case 3

A 54 year-old woman developed the sudden onset of difficulty seeing on the left side. In the ER, her EKG showed new onset atrial fibrillation. Four months later, she reported headaches and an MRI scan was repeated.

Posterior Cerebral Artery (PCA) Infarction: Flair axial MRIs 4 months after the initial stroke. Note that the bright signal has been replaced by a dark signal. This represents encephalomalacia from the earlier stroke. The tissue is physically gone and replaced by a cystic cavity, which is filled with CSF; hence the dark (low) signal on Flair imaging.
