A 55 year-old man presented with a sudden headache associated with nausea and vomiting.

Primary Intraventricular Hemorrhage. Axial CT scans without contrast. Note the intraventricular hemorrhage in the lateral, third and fourth ventricles. Most often, intraventricular hemorrhage occurs in the setting of subarachnoid hemorrhage and diffusion of blood into the ventricles. In other cases, it occurs as a primary phenomenon, presumably from a bleeding source near the ventricular lining.
