MCA Branch Infarction - Case 1

A 55 year-old woman with atrial fibrillation presented with the acute onset of an expressive aphasia, without weakness.

Outline the Infarction

Middle Cerebral Artery (MCA) Branch Infarction: Flair axial MRIs. Note the high signal in the distribution of one MCA branch involving the left inferior frontal area. This lesion resulted in an expressive aphasia. The precentral gyrus, which is just posterior to this area, was spared. Hence, the patient had no weakness. This picture of a single MCA branch infarction is nearly always secondary to an embolus.
