Infarction of Multiple MCA Branches

A 54 year-old man with right carotid stenosis presented with new left sided weakness.

Outline the Infarctions

Multiple Middle Cerebral Artery (MCA) Branch Infarctions. Flair axial MRIs. Note the bright signal in the distribution of multiple MCA branches in the right frontal, parietal and posterior parietal areas. The infarct seen on the far right image is directly on the precentral gyrus (motor strip), thus accounting for the patient's weakness. Unlike multiple cardiac emboli that typically affect multiple vascular distributions, this patient had multiple strokes all in the same vascular distribution (i.e., internal carotid artery / MCA).
