Basilar Occlusion Treated with tPA

A 41 year-old woman presented with an acute quadriparesis and dysphagia requiring urgent intubation.

Outline the Missing Basilar

Basilar Occlusion: Pre-tPA: Cerebral angiogram, right vertebral injection; (Left) AP view; (Right) Lateral view. Note the abrupt cutoff of the proximal basilar artery, with no visible branches off the basilar artery. This picture is consistent with an embolus to the basilar artery which resulted in the bilateral brainstem and cerebellar infarctions. The occipital lobes, supplied by the posterior cerebral arteries, were spared presumably because of the intact collateral circulation via the circle of Willis. This lesion is typically lethal.
