  • Make meeting Part 4 MOC requirements easier

  • Bring Northeast Ohio family physicians together to learn from each other and to and work together to improve the health care and health of the people of Northeast Ohio

  • Inform the future of maintenance of certification to make it more useful to improving care


  • Support offered by the Peer Support Collaborative Team

    • Consultation on choosing, implementing and evaluating a practice improvement project
    • Help with doing the required practice assessment.

    Options include:

  • General assessment based on your reviewing 10 of your medical records for quality in the domains of chronic illness, prevention, or acute care
  • A patient experience assessment

    -includes patient enablement, access, comprehensiveness, coordination, relationship, and suggestions on how to improve your practice.

    -We can support you in surveying the small sample of patients and can provide a report back to you.

  • Link participating physicians with others interested in working together

  • Organize planning and learning meetings in practices, groups, systems, regions, social networks, etc.

  • Furnish appropriate target dates for each phase of MOC Part 4 projects
    Host a website for communication, requested resources and free access to engaging, case-based practice improvement learning

  • Facilitate the submission of documentation to ABFM re MOC Part 4 and CME requirements
