New Hire Paperwork Appointment
Once you receive your offer letter, please contact our HR Service Center at 216.368.6964 or askHR@case.edu to complete your new hire paperwork on or before the start date according to your employment letter.
Please complete the appropriate new hire paperwork packet below (except page 2 of the federal I9 Form) on or before your first day of work or your first day will be delayed. Please bring your completed new hire paperwork, a copy of your offer letter, and I-9 documentation to Crawford, 320.

Now that your new hire paperwork is completed and you've officially started with CWRU, please activate your network ID, sign up for parking (if necessary), set up direct deposit, elect your benefits, and review HR policy and procedure.

COVID-19 Vaccine and Booster Information
If you are vaccinated, upload your card by following the instructions here. You do not need to wait until both shots are done but can upload even if you have only received one shot so far. Employees must upload proof of vaccination and proof of the booster in order to be compliant with the university's COVID protocols unless one of the exemptions above have been approved. This may mean uploading two different cards if the proof of the booster is not on the same card as the initial dose(s).

If you want to apply for an accommodation (disability or religious/ethical) from the Equity Office, that information is found here. You can also call or email the Office of Equity for more information about accommodations at 216.368-3066; firstname.lastname@example.org. The religious exemption is not limited to official religions or religious practices but will be considered for those with sincerely held ethical or moral beliefs.

Employee Assistance Program
CWRU is pleased to offer IMPACT Solutions, the university's Employee Assistance and Work/Life Program. IMPACT Solutions provides free, confidential counseling and referrals to benefits-eligible faculty and staff, as well as their family members and household members.
The program covers a wide spectrum of personal and work related challenges and daily living matters—from stress/family issues to depression, anxiety, substance abuse, legal and financial pressures, child and elder-care needs, and other commonly experienced concerns.
Professional phone support is available around the clock on an unlimited basis for you and your family members. Just call 1-800-227-6007. Provide code 'cwrueap'.Your call will be immediately answered by a qualified masters/doctoral level mental health professional 24 hours a day, 365 days a year. IMPACT Solutions has 835 providers in Cuyahoga County and 3,907 in the surrounding counties.
Face-to-face counseling is also available, with up to five free sessions included per problem occurrence.
