COVID-19 CARE Reports

If this is an emergency or a potentially dangerous situation, call Case Western Reserve University Police immediately at 216.368.3333.

Case Western Reserve aims to create a community where every member cares for one another.

During this challenging time, part of that care includes following all public health and university guidelines to help prevent the spread of infection.

Through this online form, students, parents, faculty and staff can identify students who:

  • may need additional support;
  • have symptoms/exposures; or
  • may be having difficulty adhering to public health guidelines such as wearing a mask or social distancing.

Based on the type of support needed, we connect students to members on the COVID-19 CARE Team to ensure they receive the help they need.

COVID-19 CARE Report Information

Completed COVID-19 Campus Assessment, Referral and Engagement (CARE) Reports go directly to the Dean of Students Office. They are only reviewed during normal business hours—Monday through Friday from 8:30 a.m. until 5 p.m.

CARE Reports submitted after hours, weekends or holidays will be reviewed on the next business day.

Again, for emergencies, call Case Western Reserve University Police immediately at 216.368.3333.

All reports will be handled in a confidential manner whenever possible.

The Office of the Dean of Students will acknowledge receipt of all reports within 24 hours and may contact you to obtain additional information. You may contact the Dean of Students' office at any time if you have questions about how the report is being handled. Please note that privacy concerns may prevent disclosure of specific details regarding the report.

Because students’ concerns and needs are often multi-faceted, the COVID-19 CARE Team is interdisciplinary. Members include individuals with experience and expertise in general student affairs, medical and mental health evaluation, academic support, disability resources, and residence life.

If you prefer to contact the Office of the Dean of Students to share your concern, please call 216.368.1527 or email us at
