If this is an emergency or a potentially dangerous situation, call Case Western Reserve University Police immediately at 216.368.3333.
NOTE: Please do not use this form to report your exposure, symptoms, or positive test result.
For positive tests, log into myhealthconnect.case.edu and select "Send or Read a Secure Message", then click "New Message", then select "I want to report a positive COVID-19 test". You will receive follow-up from Health Services in 24-36 hours.
For exposures, please follow the guidance here: https://case.edu/covid19/health-safety/infection-and-exposure-processes. Individuals who are exposed do not need to quarantine; they should wear a well-fitting mask and monitor for symptoms for 10 days and take a COVID-19 test at least 5 days after the exposure.
Students with symptoms should make an appointment at Health Services (myhealthconnect.case.edu) for evaluation and testing. For medical advice, call 216.368.2450. For medical emergencies, call 216.368.3333 (on campus) or 911 (off campus).
Through this online form, students, parents, faculty and staff can identify students who:
- may need additional support;
- may be having difficulty adhering to public health guidelines such as wearing a mask or social distancing.
Based on the type of support needed, we connect students to members on the COVID-19 CARE Team to ensure they receive the help they need.
