Financial Aid Forms

Completing Your Financial Aid Application


Below you'll find the most common forms in use by the Office of University Financial Aid. Some of these are required by the office as part of a student's initial application for financial aid. Others are requested by the office on a case-by-case basis as documentation of special circumstances.

All information provided to the Office of University Financial Aid by the student, or by the student's parents or spouse, is completely confidential.

Although there are a large number of forms listed below, only a small number will apply to a given student. Please do not submit all these forms. As a general rule, if you don't know what the form is, or you're not certain it applies to you, please do not submit it.

These forms pertain to the 2018-2019 academic year only. If you have any questions regarding the type of forms you should fill out or need forms for the 2017-2018 academic year, please do not hesitate to contact us.


CWRU Financial Aid Form

The CWRU Financial Aid form is required for all enrolling students. The form is available online through the My Financial Aid portal. In order to complete the form, log in using your CWRU Network ID.

Free Application for Federal Student Aid (FAFSA)

The FAFSA is the primary financial aid form for CWRU and most other universities across the country. The U.S. Department of Education sponsors the FAFSA form and requires its use by any educational institution disbursing federal financial aid.

Request for Refund Advance

Complete this form and submit it to the Financial Aid Office if you are requesting an advance on your financial aid refund for the fall of 2017. This form is valid only for students enrolling or enrolled in the following schools: Undergraduate Colleges, School of Graduate Studies, Frances Payne Bolton School of Nursing, Weatherhead School of Management and the Mandel School of Applied Social Science.

High School Completion Status

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If you are notified by our office to submit this form, your record has been selected and you must submit the High School Completion Status form.

IRS Tax Forms

If a student is selected for verification by the U.S. Department of Education or CWRU, he or she must submit a copy of the complete IRS tax transcript and W-2 forms to the Office of University Financial Aid. Applicants for the 2018–19 academic year should submit the forms from tax year 2016.

Which tax transcripts?

  • Dependent students should submit copies of the parents' IRS Tax Return Transcript. 
  • When parents file separately, the Tax Return Transcripts for both parents must be submitted.
  • Married students filing separately should submit copies of the spouse's IRS Tax Return Transcript.

How?

Families are encouraged to utilize the Data Retrieval Tool on the FAFSA website when completing the FAFSA and not change any of the transferred figures. This will eliminate the need to obtain copies of the IRS Tax Return Transcripts.

If the Data Retrieval Tool cannot be used, the IRS Tax Transcript can be obtained at irs.gov. On the homepage under Tools, choose Get a Tax Transcript. Follow the instructions for online ordering. 

Submit the forms in their entirety. The tax transcripts and W-2 forms require submission of the cover page for each document. Only one cover page is required each time document(s) are submitted.

If You Did Not File

Students or parents who have not, will not and are not required to file a Federal Income Tax Return will need to show documentation from the IRS that they did not file a 2016 tax return. Please refer to the Verification of Non-Filing Form below.

Memorandum of Assistance (MOA)

The Memo of Assistance form is required of all graduate and professional students applying for financial aid. Unlike most other forms, it is not completed by the student. Instead, it is completed by the graduate department for the program in which the student is enrolled. A graduate department representative completes the form and forwards it directly to the Office of University Financial Aid, certifying the following information:

  • Details of the degree and program
  • Exact number of credit hours and tuition fees charged for the academic year
  • Type and amount (if any) of graduate financial assistance being received by the student through the department in question

This form is required of all graduate and professional students applying for aid at CWRU, even if said student is not receiving any financial assistance from his or her department. If you are not certain that the form is going to be sent by the university department in question to the Office of University Financial Aid, please contact the department to arrange this.

Proof of Citizenship

The Department of Education requires our office to verify some students' citizenship status. You are required to submit appropriate documentation, most often a birth certificate, certificate of naturalization or a valid U.S. passport, along with this document.

Proof of Permanent Residency

The Department of Education requires our office to verify some students' Permanent Residency status. You are required to submit appropriate documentation, most often a copy of your United States Permanent Resident Card (USCIS I-551), along with this document.

Proof of Selective Service Registration

The Department of Education requires our office to verify male students' Selective Service registration. You are required to submit a copy of your Selective Service registration card or registration proof from the Selective Service website, sss.gov, along with this document.

Statement of Educational Purpose and Proof of Idenity

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If you are notified by our office to submit this form, your record has been selected and you must submit the Statement of Educational Purpose/Proof of Identity form. This form must be brought into the Office of University Financial Aid in person; if a student cannot bring it in person, this statement must be notarized by a notary.

Student and Parent Untaxed Income Worksheet

The Untaxed Income Worksheet is not required as an initial part of the student's application for financial aid. If there is a question regarding untaxed income received by a student or parent, this form may be requested by the Office of University Financial Aid. Please note the student's signature is required, even if it was the parent who reported untaxed income.

Study Abroad Cost Sheet

This form should be completed by students participating in the Study Abroad Program.

Verification Form – Dependent Students

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If a student is selected, he or she will have this document listed as a required document.

Students requiring a Verification form should review the information (along with their parent(s)), and then complete and return it, as indicated on page 1. As part of the verification process, students and their parent(s) are required to submit copies of their 2016 IRS tax transcript and W-2 forms as supporting documentation. Please see Internal Revenue Service forms above.

Verification Form – Independent Students

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If a student is selected, he or she will have this document listed as a required document. Independent students requiring a Verification form should review the information (along with his/her spouse, if applicable), and then complete and return it, as indicated on page 1. As part of the verification process, students (and their spouses) are required to submit copies of their 2016 IRS tax transcript and W-2 forms as supporting documentation. Verification Form - Non-Filing

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If the student or his/her parent(s) did not, will not and are not required to file a 2016 income tax return, this form is required. With this form, families in this situation must provide documentation from the IRS (or other tax authority if outside the U.S.) of the non-filing status.
