Sign up for biometric screening, complete wellness forms to receive financial incentives

Don’t miss your chance to receive at least $300 in 2016 with the Wellness Incentive program. To receive the financial incentives, employees enrolled in one of the university’s medical insurance plans must complete a biometric screening, Health Risk Appraisal and Tobacco Attestation Form.

Biometric Screening

Less than two weeks are left to schedule an on-site biometric screening this fall; the deadline to make an appointment is Friday, Nov. 13. Appointments are available from the middle to the end of November on campus. Registration is now available for the biometric screening at We.BlueprintForWellness.com. Use registration key “CWRU” and sign up for one of the on-campus screenings offered through Nov. 30 in three campus locations: Tinkham Veale University Center, Crawford Hall and the Frances Payne Bolton School of Nursing. Registration closes two weeks prior to each screening day. Sign up now to make sure that you get this important step completed. Completing the biometric screening will give you helpful information about your health. The free screenings include blood pressure, height, weight, waist circumference, cholesterol and glucose tests. Fasting for nine to 12 hours in advance is necessary. If none of the on-campus screening dates are convenient for you, visit the registration site to find a Patient Service Center location in the Cleveland area. If you have had all of the above screening tests completed since April 1, you may use this form (PDF) to share your results with Quest. The form must be signed by your health care provider and faxed to Quest prior to Nov. 30 to count toward this participation requirement. Individual results will remain confidential. Health Risk Appraisal (HRA)

Completion of the Health Risk Appraisal is required by Nov. 30. The University of Michigan administers this comprehensive, private health questionnaire online. A personalized health risk profile is created following completion of the questionnaire and the biometric screening process. A secure process is used to include biometric screening data within the HRA. Faculty and staff will receive a confidential results report. The university cannot access individualized data; it only receives a de-identified aggregate report. The HRA is available until Nov. 30 at case.edu/hra. Any benefits-eligible employee may complete this form; however only those enrolled in a medical plan during 2016 may be eligible for the monthly incentive.

Tobacco Attestation Form

Faculty and staff who wish to receive the monthly Wellness Premium Incentive during 2016 must complete the Tobacco Attestation Form by Nov. 30 at case.edu/hcm.
