Ann Reichsman, clinical associate professor at Case Western Reserve University School of Medicine, recently had an article published in Annals of Family Medicine.
Titled “The Warmest of Handoffs: A Neighborhood Physician’s Transfer of Care,” Reichsman wrote about the story of transitioning patient care from a retiring family physician to her recently trained replacement.
