Both forms must be filled out completely and legibly before your registration can be processed. Completed forms may be submitted via fax, or hard copies may be mailed to the following address:

Fees
CAPSC course audit participants are charged $40 per credit hour for members of Siegal Lifelong Learning; $50 per credit hour for nonmembers. Some courses that have extra activities may incur additional fees for these optional activities.
