General

Academic Advisement Report Substitution Form – Use this form to request a correction or course substitution on your Academic Advisement Report. Have form reviewed by your advisor and send to the Dean assigned to your alphabetical cohort, determined by the first letter of your last name (N. DiIulio= A-Ho; A. McCarthy= Hp-Ph; D. Poulos= Pi-Z).

Cross-registration Form – Use this form to register for a class via the Cross-registration Program. The form must be submitted prior to the start of the semester. Send to ugstudies@case.edu.

‌Dean's Verification Form – Use this form to request a letter from the dean's office verifying your academic standing. Send to ugstudies@case.edu.

Major Declaration/Change Form – Use this form to declare or change a major. Complete form, send to Academic Representative, then send to ugstudies@case.edu

Minor Declaration/Change Form – Use this form to declare or change a minor.‌ Complete form, send to Academic Representative, then send to ugstudies@case.edu

Planned Program of Study Form – Students applying for the BS/MS or IGS programs must submit his form with their application materials.‌ Send to UG Studies with approval of your graduate program advisor.

Drop/Add Form – Use this form to drop or add a class to your schedule. Send to your Navigator for review.

Online Submission

Application for Graduation – Use this online form to apply for graduation through SIS (currently closed). If you need to apply for graduation and the online SIS form has closed, you will need to complete a paper Graduation Application and submit to ugstudies@case.edu.

Petitions and Requests

Off-campus Study Request Form – Use this form to request approval to take a course off-campus. Prior approval is required in order for transfer credit to be granted. Send to ugstudies@case.edu.

Petition/Special Request – Use this form to make a special request or to petition a change to your academic record. Complete and send to the Dean assigned to your alphabetical cohort, determined by the first letter of your last name (N. DiIulio= A-Ho; A. McCarthy= Hp-Ph; D. Poulos= Pi-Z).

Petition for Re-enrollment After Separation – Use this form to request to return to CWRU after an academic separation.

Petition to Re-enroll After Voluntary Leave of Absence – Use this form to request to return to CWRU after you have taken a voluntary leave of absence.

Request for Voluntary Leave of Absence – Use this form to request a voluntary leave of absence from CWRU. It is used if you are NOT enrolled for classes during the semester in which you are requesting the leave. If you are registered for classes, use the for Withdrawal Form.

Request for Withdrawal - Use this form to request to be withdrawn from all courses in which you are currently enrolled.

Transfer and Visiting Students

Visiting Undergraduate Student Fall/Spring Application – Use this form to apply to be a transient/visiting student at CWRU during the fall or spring semester. Send to ugstudies@case.edu.

‌‌Visiting Undergraduate Student Summer Application – Use this form to apply to be a transient/visiting student at CWRU during the summer. Send to ugstudies@case.edu.

Visiting International Student Application (non-exchange) – Use this form if you are an international student and want to apply to be a transient/visiting student at CWRU during the summer, fall, or spring term. Send to ugstudies@case.edu.
