Case Western Reserve University faculty and staff who plan to use a university medical plan next year have until Nov. 30 to complete eligible activities to qualify for a $25 Monthly Wellness Incentive in 2027.
To earn the incentive, you must complete a WebMD Health Risk Assessment, plus at least two of these four options:
- Primary Care Provider Attestation Form
- Biometric Screening Attestation Form
- Dental Check-up and/or Cancer Screening Attestation Form
- Tobacco Attestation Form
All attestation forms are available in HCM within the Wellness tile. You may need to scroll to the bottom of one of the forms to click the button to "Add Next Benefit Year" for access. Please note that the Wellness Incentive Program is voluntary, and all earnings are taxable.
Those who qualify for the $25 Monthly Wellness Incentive this fall will have the opportunity to earn an additional $200 in Program Incentives in 2027. For those who are working toward that goal this year, don't forget to attest in HCM by Nov. 30.
