Learn more about your health by completing three wellness activities this fall: a biometric screening, Health Rick Appraisal and Tobacco Attestation Form. All benefits-eligible faculty and staff can complete the screening and Health Risk Appraisal, however only employees who use one of the university’s medical insurance plans will be able to receive the $25 monthly Wellness Premium Incentive in 2016. To receive the benefit, eligible employees must complete three activities by Nov. 30.
