ACT Implementation Guide


This booklet helps organizations implement Assertive Community Treatment (ACT), the evidence-based practice. ACT improves outcomes for people with severe mental illness who are most at risk of homelessness, psychiatric crisis and hospitalization, and involvement in the criminal justice system. This document is organized as a checklist of activities in five stages of change and implementation:

  • Pre-Contemplation / Unaware or uninterested
  • Contemplation / Consensus building
  • Preparation / Motivating
  • Action / Implementing
  • Maintenance / Sustaining
