University Health and Counseling (UH&CS) adhere to strict professional standards of confidentiality and do not share your Protected Health Information (PHI) without your prior consent, except in cases of imminent danger of harm or when compelled by law.
If you would like to request a copy of your medical and/or mental health records, or would like to give permission for your provider to discuss medical and/or mental health information with a third party, please do the following:

At this time, we are providing medical and mental health records electronically through secure messaging. All requests will be processed within 2-3 weeks. If you have concerns or questions regarding this process please email uhcs@case.edu
