To schedule an appointment at either location, please call 216.844.3944 or make an appointment online.
Prescription Refills
To refill a current prescription, please have your pharmacy fax a refill request to 216.844.8946. You can also login to the secure FollowMyHealth™ website or app. Once your request is received and approved, your prescription will be refilled within 48 business hours.
Medical Records
You can view your health records online, including lab test results, using the secure online system FollowMyHealth™. To request a paper copy of your medical records please complete and submit a Medical Release Form. The form can be submitted by dropping it off at the office, or by fax at 216.844.8591. Please allow 7 to 10 business days for the completion of your request. If you need to contact medical records, please call 216.844.8518 to speak directly with our medical records staff.
