Forms and Submission

This page includes forms that list the documentation required for various actions that are submitted to the office of the deputy provost.  A manager of faculty appointment matters for each school has access to the forms on this site.  Faculty or staff who wish to review a form or instructions for its completion may obtain a copy from the manager.

The following actions are also submitted to the office of the deputy provost:

  • *Appointment of full-time lecturers
  • *Transfer of faculty appointment from one school to another
  • *Secondary appointments across schools (initial appointment or promotion in rank)
