Many forms commonly requested by the Office of Student Employment are available below for viewing or downloading. All information submitted on these forms is kept confidential and must be submitted in person to Sears 240. If you have any questions or concerns about which forms to fill out or any other documentation requested by our office, please contact us.
Forms Required of All Student Employees
- Federal Form I-9
- Federal form I-9 is used to verify the identity and employment authorization of individuals hired for employment in the United States. Instructions are available here.
- Federal Form W-4
- Federal Form W-4 is used to withhold the correct amount of federal income tax from your pay.
- Ohio IT-4
- State Form IT-4 is used to withhold the correct amount of Ohio income tax from your pay.
- 2026–2027 Student Acknowledgment Form (Submit online via OnBase)
- CWRU Form used to acknowledge a student's intent to work and comply with all federal, state, and university employment requirements.
- Note on OnBase: Your OnBase account will be created by the end of the next business day after your hire form is received. If you experience login issues, please complete the linked Account Creation Form. More information is available in this UTech Knowledge Base article.
