Student Hiring Department Forms

Many forms commonly requested by the Office of Student Employment are available below for viewing or downloading. All information submitted on these forms is kept completely confidential. If you have any questions or concerns about which forms to fill out or any other documentation requested by out office, please contact us.

HCM only allows one individual to be listed on a student’s job record. First-time and additional individuals needing access to view or approve a student’s hours must have their department update its security options in HCM to allow additional access. Instructions on that process can be found on UTech’s HCM Knowledge Base page under Department Security Entry.

Forms required of all hiring departments

  • Academic Year 2024-2025 Hire/Rehire Employment Form
    • If access to this form is required, email the UTech Service Desk to have access granted. This form can be completed by anyone at the department authorized to hire students or approve their time and must be started by the department. The person filling out the form should be the same person who signs the form.
  • Maintenance Form
    • This form reports changes to employment information after a student has been hired. It should be submitted to sehireforms@case.edu.
  • Payment Request Form
    • This form is to request a one-time lump-sum payment for a student and is used when the student completes a short project (30 days or less) and will not continue working for that supervisor. It should be submitted to sehireforms@case.edu.
