The Swetland Center is partnering with Better Health Partnership and Produce Perks Midwest to design and facilitate an annual workshop to support local clinical sites that want to create a new produce prescription program or enhance their existing program. The goal is to leverage statewide and local grant and funding expertise to help local produce prescription program providers identify funding sources.

The REACH project is funded by a grant (#6 NU58DP007747-01-01) from the Centers for Disease Control and Prevention, through the Cuyahoga County Board of Health. Our partners on the project are Better Health Partnership, Catalytic Change Link, Cleveland Fresh LLC, and Produce Perks Midwest. This project is funded from December 30, 2023 to September 29, 2028.

During this free, 1-hour technical assistance session, your organization will receive tailored guidance from either Better Health Partnership or Produce Perks Midwest. The goal of this session is to help your organization start a new produce prescription program, or adapt or expand your current program. The facilitators will connect you to funding sources and other support.
