Review Period: 5 years
Date of Last Review: August 6, 2025

Relates to: Undergraduate, Graduate, and Professional Students

This policy pertains to undergraduate, graduate and professional students responsible for university disciplinary policy violations. This includes but is not limited to violations of the University Student Code of Conduct, the Undergraduate Academic Integrity Policy, the Graduate Academic Integrity Policy, and the University Sexual Misconduct Policy. Policy violations will lead to the investigating office creating a disciplinary record and reporting the violations as follows:

  • Any disciplinary violation that leads to sanctions less severe than disciplinary probation, university separation or university expulsion will only be taken into consideration when addressing any subsequent disciplinary violations. Such violations will be considered non-reportable to other higher education institutions or agencies seeking information about student disciplinary records.
  • Any disciplinary violation that leads to disciplinary probation or university separation, which are both for a set period of time with a specified end date, will be reportable for eight years from the date of the incident.
  • Any disciplinary policy violation that leads to a sanction of university expulsion, which is a permanent separation from the University, will remain on record with the University permanently and be permanently reportable.
  • Any first offense academic integrity policy violation will be non-reportable and will only be taken into consideration when addressing any subsequent academic integrity violations.
  • In cases where a student is alleged to have violated a policy but is found not responsible, the University will retain a record of the finding, but the finding will be non-reportable to other higher education institutions or agencies seeking information about student disciplinary records.

Students should be aware that higher education institutions, licensing authorities (medical, legal, etc), government agencies, or other organizations may request information on ANY and ALL student policy violations for which the University found the student responsible. Students are advised to respond honestly to these requests, even if the University considers the violation non-reportable as described in this policy.

This policy is effective May 1, 2016. Any question of interpretation or application of this policy shall be referred to the Vice President of Student Affairs or a designee for final determination.
