The Biometric Screening Attestation requires you to honestly attest that you have completed, or will complete, a Biometric Screening with your Primary Care Provider (PCP) during the program year.
A Biometric Screening is defined as a blood test ordered by a Primary Care Provider (PCP), such as a Medical Doctor (MD), Doctor of Osteopathic Medine (DO), Nurse Practitioner (NP), or Physician Assistant (PA). The screening should include, at a minimum, height, weight, glucose, cholesterol, triglycerides, and blood pressure, and possibly waist circumference. PCPs use screening data to assess and evaluate health risks. This preventive care is important to monitor your health and to ensure that you receive the right care at the right time, based on your current health status.
The attestation forms associated with the Monthly Wellness Incentive are accessible in the Wellness tile of HCM from September 1 through November 30 annually. For those newly hired to the university, there are paper copies available in their new hire orientation packet.
Filling out the form should take approximately one to three minutes. Additional time requirements include scheduling and attending the appointment with your primary care provider.

Due to budget constraints at the university, the Wellness Program no longer offers biometric screenings through Quest Diagnostics. However, this presents an opportunity for us to encourage and incentivize employees completing this service with their primary care provider (PCP). Numerous benefits are associated with having an established primary care provider. Regular visits with a PCP encourage preventive exam compliance (e.g., colonoscopy, mammograms, etc.) and also lead to increased rates of necessary immunizations.
