To become a Wellness Participant and qualify for the $25 Monthly Wellness Incentive in 2025, you must complete the following checklist between September 1 and November 30, 2024.
- Enroll in a Benelect medical plan (or planning to do so for following calendar year) Check eligibility.
- Complete the Health Risk Assessment (HRA) on-line with WebMD on or before November 30, 2024.
- Complete TWO of these THREE activities:
- Submit the Tobacco Attestation Form. Go to HCM then choose the Wellness Tile. Choose the Tobacco Attestation Form from the menu on the left. Complete it honestly and submit by November 30, 2024.
and/or
- Submit the Primary Care Provider Attestation Form, Go to HCM then choose the Wellness Tile and select the Primary Care Provider Form from the menu on the left. Complete it honestly and submit it by November 30, 2024.
and /or
- Complete a Biometric Screening by November 30, 2024. Schedule and complete an appointment at one of the Quest Diagnostics Patient Service Centers (create an account using registration code "CWRU" or log in as previous user), at an on-campus screening event, or have your physician complete the Physician Results Form.
All standard HIPAA rules apply.
To earn a program incentive of (up to) $200 in 2024, you must complete the following checklist by November 30, 2024.
- Currently be a Wellness Participant (receiving $25 monthly wellness earnings in paycheck).
- Select a Program in one or two of the nine Wellness Program categories: Career Well-being, Community Well-being, Financial Well-being, General Wellness, Nutrition or Weight Management, Physical Activity, Sleep, Stress Management, and/or Tobacco Cessation.
- Satisfy the guidelines for completion in each program.
- Record completion through the attestation form found on HCM.
Participants are encouraged to check their status anytime in HCM. Choose the Wellness tile, then select the Wellness Summary from the menu on the left. You will see current, prior, and future benefit year status.

This applies only to those who start between December 1 and June 15 annually. All those who begin after June 15 will be invited to participate in the fall with the current faculty and staff.

Applies to new benefits-eligible employees beginning employment between December 1, 2023 - June 15, 2024, who are able to complete all required activities to earn the $25 Monthly Wellness Incentive by the 30-day deadline. All new employees joining the university between June 15 and November 30, 2024, may complete the three wellness activities during the fall enrollment timeframe (along with all other current benefits-eligible employees) to be eligible for wellness incentive monies in 2025.

Participation Guidelines
The 2024 Wellness Program Incentive is available to any Wellness Participant who, between January 1 and November 30, 2024, successfully completes the program requirements in up to two of the following topics/areas:
Participating in professional development programs and activities, offered within or external to the university, will meet the participation guidelines for this incentive. Regular participation over a multi-week period (attending at least 85% of sessions over a minimum of 7-8 weeks), is necessary to be eligible for this incentive.
Participating in a university-based community effort and attending 85% of sessions for a minimum of 8 weeks will meet the participation guidelines for this incentive.
Participating in a multi-session program to learn more about financial management and/or healthcare consumerism and attending 85% of available sessions will meet the participation guidelines for this incentive.
Participating in a university-based general wellness program over a multi-week period (attending at least 85% of sessions over a minimum of 8 weeks) is necessary to be eligible for this incentive.
Participation possibilities include attending an on-site or off-site multi-session weight management program such as Weight Watchers (multi-week program, attending 85% of available sessions), and/or working with your health care provider on a documented weight management program over a multi-week time period.
Participants must attest and certify to participation in an on- or off-site organized weight management or nutrition program activity that maintains participation records available for review upon request; a comparable program with a health care provider and with personal records documenting participation available for review; or a program that meets the Reasonable Alternative Standard, as confirmed by written pre-approval of the CWRU Medical Director.
Engage in regular physical activity over a minimum of 12 weeks (minimum of three moderate-intensity activity sessions, each lasting at least 30 minutes per week).
Activities may include any planned exercise including but not limited to the following:
- exercise sessions at university fitness centers (121 Fitness, Wyant Center, Veale Center)
- yoga
- Pilates
- walking
- training for a 5K, 10K, or other race
- bicycling to work
- achieve 300 points in Worldwide Wellness or similar 12-week exercise incentive campaign
Documentation of activity required. Types of documentation may include: use of activity tracking devices such as FitBit, Jawbone, an accelerometer or pedometer; paper log; print out of visits from fitness or yoga center; etc.
Programs must be successfully completed and attestation submitted by November 30th, 2021 to receive the program financial incentive.

Participating in an online, on-site, or off-site tobacco cessation program (multi-week program, attending at least 85% of available sessions) is necessary to be eligible for the incentive in this category. Non-tobacco users are not eligible for this incentive.
HCM Documentation
Participants will need to complete a form within HCM to attest and certify to participation in on-site or off-site programs that met the university’s participation guidelines and must have documentation of their participation available should we ever need to audit incentive recipients. Completion of a program that meets the Reasonable Alternative Standard as confirmed by written pre-approval of the CWRU Medical Director is also available. The attestation form can be found within the Wellness Tile at case.edu/hcm.
Programs must be successfully completed and attestations submitted by November 30, 2024, to receive the program financial incentive in the final December 2024 paycheck.
