Dental Check-up and/or Cancer Screening Attestation

The Dental Check-up and/or Cancer Screening Attestation Form requires you to honestly attest that you had, or will have, a dental check-up and/or cancer screening at least once during the incentive program year. 

A dental check-up is defined as an assessment of the mouth and teeth to identify cavities, gum disease, and/or oral cancer. Twice a year (every six months) visits to a dentist are recommended by the American Dental Association.

Specific factors, such as age, gender, and other factors influence the type of cancer screenings recommended for individuals. Professional organizations, such as the American Cancer Society, and the Centers for Disease Control and Prevention (CDC) recommend screenings for breast, cervical, colorectal, and lung cancers.

The attestation forms associated with the Monthly Wellness Incentive are accessible in the Wellness tile of HCM from September 1 through November 30 annually. For those newly hired to the university, there are paper copies available in their new hire orientation packet. 

Filling out the form should take approximately one to three minutes. Additional time requirements include scheduling and attending the appointment with your primary care provider(s).

The CWRU Benelect provider, Medical Mutual, offers help to locate a provider close to your home or the office.

The Dental Check-up and/or Cancer Screening Attestation requires you to honestly attest that you had, or will have, a dental check-up and/OR cancer screening at least once during the program year. Completing just one of these activities is necessary to attest with this form. While both services are recommended for general well-being, only one is needed for credit toward the incentive program.
