In addition to the required Health Risk Assessment, the Dental and/or Cancer Screening Attestation is one of the four options (along with tobacco attestation, primary care provider attestation, and/or biometric screening attestation; participants choose at least two of the four activities to complete) that are offered for faculty and staff who want to earn the Monthly Wellness Incentive.
The Dental and/or Cancer Screening Attestation form can be found on the Wellness Tile within HCM between September 1 and November 30 annually. New hires to the university should complete the paper form included in their orientation packet.
Dental and/or Cancer Screening Attestation
A dental screening is defined as an assessment of the mouth and teeth to identify cavities, gum disease and/or oral cancer. Twice a year (every six months) visits to a dentist are recommended by the American Dental Association.
Specific factors, such as age, gender, and other factors influence the type of cancer screenings recommended for individuals. Professional organizations, such as the American Cancer Society, and the Centers for Disease Control and Prevention (CDC) recommend screenings for breast, cervical, colorectal, and lung cancers.
The Dental and/or Cancer Screening Attestation requires you to honestly attest that you had, or will have, a dental and/or cancer screening at least once during the program year. Completing just one of these activities is necessary to attest with this form. While both services are recommended, only one is needed for credit toward the incentive program.
