For New Students

University Health & Counseling Services provides a range of services including primary care, sexual health, travel care, nutrition, individual and group counseling. There is no cost to see a provider, regardless of what health insurance you have, if you are a student registered for at least one credit hour.

We are located in the Dental Research Building, 1st floor, at 2124 Cornell Rd. Entrance is on podium level of building across from BRB cafeteria. 

Requirements

Non-Health Science Students

Examples of Non-health Related Programs include:

  • Case Western Reserve University Undergraduate
  • Case Western Reserve University Graduate
  • Cleveland Institute of Art
  • Cleveland Institute of Music

Forms for students enrolling for the fall semester should be submitted by July 1 or two weeks before your program begins. For the spring semester, forms should be submitted by January 1.

While most students (with the exception of nursing and other health science students) are not required to have received any immunizations, we highly encourage students to be up-to-date on ALL recommended immunizations, especially MMR (Measles, Mumps, Rubella), Meningitis, and Hepatitis B.

Log in to myhealthconnect.case.edu and click on "Medical Clearance". Complete the following online forms:

  1. Acknowledge the Privacy Statement
  2. Complete the Health History Form
  3. Enter Immunization History
    • For each immunization record you have, click on update next to that line
    • In the pop-up box, click on “add immunization” enter date immunization was received along with immunization name. 
    • Repeat this process until you have entered all immunization information
    • Once done, click on black “Add immunization record” at top of screen and upload supporting documentation.
    • You may need to contact your health care provider to obtain your immunization history.
    • If you have questions regarding your immunization records or what is required, send a secure message to “Nurse, Immunization” in myhealthconnect.case.edu. 
      • Click on Messages in left hand menu
      • Select new message
      • Select “I want to send a message to the immunization nurse”
  4. Enter Insurance Information **If you are planning on taking the Aetna Student Health Insurance you do not need to complete this step** While most services at UHCS are provided at no-cost, you may need to use your medical plan for an x-ray, lab test, prescription or visit to the Emergency Room.
    • To enter insurance information: 
    • Log-in to myhealthconnect.case.edu
    • Click on Insurance card tab on left hand menu
    • Select “add new card”
    • Enter information from your insurance card  into fields on the left hand side of the pop-up. It is ok if you do not have all the information– most important fields are Member Name, Member ID, and Plan Provider (Name of Insurance Company). 
    • Upload a copy of the front and back of your card by clicking on the green Upload front/back image. 
  5. Complete the insurance waiver: 
    • Review the insurance waiver criteria on the UHCS website
    • You are responsible for ensuring your insurance plan meets this criteria. 
    • Students studying in OHIO must ensure that their plan covers them in OHIO. Most Kaiser, out of state medicaid and international travel plans do not appropriately cover healthcare in Ohio. If you attest that your plan covers you, you will be responsible for covering any costs not covered by your insurance plan. 
    • Follow prompts to complete waiver. You will be asked to upload your card again.

Cleveland Institute of Art Students

Prior to logging in to myhealthconnect.case.edu you must activate your CWRU Network ID. You should have received an email with instructions. If you did not, you can find instructions on the Utech website or by calling the help desk number above. 

New Students- Health Related Programs

  • Lerner School of Medicine
  • Case School of Dental Medicine
  • Frances Payne Bolton School of Nursing
  • Physician Assistant Program
  • Kent State University School of Podiatric Medicine
  • Masters Program in Anesthesia
  • Public Health Nutrition Dietetic Internship
  1. Review your school's immunization requirements, then contact your health care provider to obtain personal immunization information. Students who are in these programs can use this Health Science students vaccination worksheet to help gather information.

Health Services does not provide drug screening however, we have partnered with CastleBranch to offer drug screening to the university. More information can be found on the Occupational Health webpage.
