The Health Risk Assessment (HRA) is a free, confidential questionnaire about your diet, exercise, smoking, drinking and other lifestyle behaviors. Everyone who completes the HRA will be able to obtain a confidential, individualized report.
The HRA is available online annually by mid August. The university will only receive de-identified, aggregated data regarding its overall workforce and will not have access to any individual responses.
Any benefits-eligible faculty or staff member may complete the assessment; however, only those enrolled in a medical plan through CWRU are eligible for the incentives.
During the fall of each year, faculty and staff are encouraged to complete a free and confidential online Health Risk Assessment (HRA) by visiting the HRA online.

If you complete a Quest Diagnostic health screening, the data, such as blood pressure and total cholesterol, will be rolled into your survey within 48-72 hours of your completed screening.
Please Note: Once you complete the HRA, it can take from 10-14 days for it to be recognized in HCM. However, you will be credited for completion on the day you take the assessment.
Each individual who completes the assessment will receive confidential, personalized feedback regarding the results. Completion of the HRA is one necessary step to obtain the Monthly Wellness Incentive each year. The deadline to complete the HRA is November 30 each fall. While no one at the university will see any single person’s responses*, program officials will receive de-identified, aggregated data to identify trends and opportunities involving the campus community. This information will be used to guide decisions regarding future program offerings to enhance well-being and help prevent chronic disease and acute illness.
