Vaccination and Booster Verification

As part of our efforts to protect the health and safety of the campus community, Case Western Reserve is asking everyone to confirm their vaccine and booster status via the university’s secure health record system.

Below are step-by-step instructions for both the booster dose and primary vaccination processes.

If you lost or misplaced your CDC card and received your COVID-19 vaccine in Ohio, please complete this form to request a duplicate card. If you received your COVID-19 vaccine outside Ohio, please contact your state’s department of health to get an official vaccination record or a new card.

Booster Dose

To comply with the university’s booster requirement, members of the campus community must:

  • receive a booster dose on campus, where it will be documented automatically in the university’s secure health record system; or
  • receive a booster dose off campus, and provide documentation to the university; or
  • secure a religious or medical exemption from the Office of Equity (medical for faculty and staff, religious for all members of the campus community) or Office of Disability Resources (medical for students).

Booster Dose Verification Instructions

To be used only if you received your booster off campus.

Below are step-by-step instructions to access the platform and provide necessary information. You will need to upload a photograph or scan of your vaccination documentation (typically a card) to complete the verification process.

Please note: Your status will not immediately shift to compliant after you complete this process, nor will you receive a prompt communication verifying receipt of your upload. 

If you receive no error message after uploading the photograph of your card, presume you are now compliant.

Health Services must manually verify your status, a process slowed by limited staffing and the thousands of people required to document their booster.

Before you begin: Take a photo or scan of the front and back sides of your vaccination card and ensure you have access to the file on the device on which you're completing this form.

To submit your verification:

  1. Visit myhealthconnect.case.edu and log in using Single Sign-On (if requested).
  2. Enter your birthdate and select "Proceed."

  3. Choose "Enter my COVID-19 Vaccination Information" on the home page:

  4. Follow the directions on the next page to upload the photograph of your vaccination card.
  5. Select "Done" to finish.

Primary Dose Verification Instructions

Before you begin: Take a photo or scan of the front and back sides of your vaccination card and ensure you have access to the file on the device on which you're completing this form.

To submit your verification:

  1. Visit myhealthconnect.case.edu and log in using Single Sign-On (if requested).
  2. Enter your birthdate and select "Proceed."

  3. On the page that follows, select "Medical Clearance" from the navigation. (If you're on a desktop, this will be located on the left.)

  4. Next to "COVID," select "Update."

  5. If you haven't already, take a photo of the front and back sides of your vaccination card.
  6. Select "Upload" and choose your vaccine card image file.

  7. Enter the date(s) of your vaccine using the MM/DD/YYYY format and select your vaccine manufacturer from the dropdown menu.* If your vaccine required two doses, be sure to fill out both rows of information.

  8. Select "Done."
  9. When you've completed these steps, your status will change to "Pending Verification" or “Not Compliant.” Please do not be distressed; a nurse still needs to review your documentation before the entry here will read “Compliant.” This review process can take up to 10 business days.
  10. You can log in at a later date to confirm that your status has switched to "Compliant."

*If you received a vaccine outside the U.S., its name will not appear on the dropdown menu. Please skip that section and upload a photograph of the documentation you received. Reviews of vaccines administered outside the U.S. will take longer than domestic ones; the university will provide broad updates regarding these vaccines later this summer.
