The Biometric Screening Attestation requires you to honestly attest that you have completed, or will complete, a Biometric Screening with your Primary Care Provider (PCP) during the program year.
A Biometric Screening is defined as a blood test ordered by a Primary Care Provider (PCP), such as a Medical Doctor (MD), Doctor of Osteopathic Medine (DO), Nurse Practitioner (NP), or Physician Assistant (PA). The screening should include, at a minimum, height, weight, glucose, cholesterol, triglycerides, and blood pressure, and possibly waist circumference. PCPs use screening data to assess and evaluate health risks. This preventive care is important to monitor your health and to ensure that you receive the right care at the right time, based on your current health status.
