Benefits for Reproductive Services and Mental Health/Substance Use Disorders (including Gender Dysphoria)
Travel Benefits
The university covers travel benefits that are primarily for and essential to medical care for covered services that cannot be legally rendered in the state of the member’s residence through our medical insurance plans with Medical Mutual of Ohio (MMO). To receive these benefits, members must travel at least 100 miles from their residence for such medical care.
The medical care for covered services must be lawfully obtained in the location where the services are provided. Reimbursement of up to $1,000 per occurrence is limited to: mileage reimbursement based on current Internal Revenue Service (IRS) mileage reimbursement standards for medical travel (currently at $0.22/mile); reasonable rate or fare for bus, taxi, ride service, train or subway; reasonable rate for economy airfare; lodging of $50 per night per person up to $100 per night (any lodging for a companion is covered if the companion’s presence is necessary to enable the patient to receive medical care).
Travel benefits are subject to any cost-sharing requirements, such as deductible, coinsurance and copayments. PPO and High Deductible plan members must use a network provider. HMO plan members—e.g., those enrolled in CLE-Care must obtain prior authorization for care outside the network if no in-network provider can perform the services.
To apply for travel reimbursement, fill out MMO's Travel and Lodging Reimbursement Form.
Medical Benefits
The university’s health plans continue to prioritize your reproductive health in part by covering related preventative services without application of a Deductible, Copayment or Coinsurance,--when the services are provided by in-network providers. These services include well-woman visits; screenings for gestational diabetes, human papillomavirus (HPV), human immunodeficiency virus (HIV) and sexually transmitted disease; contraceptives and counseling for contraceptives for women with reproductive capacity; sterilization procedures; breastfeeding; and domestic violence. In addition, other services covered by our PPO and High Deductible health plans include invitro fertilization (IVF) and artificial insemination. For IVF, the university provides a $10,000 lifetime maximum of which the member pays 50% of the procedure cost and the plan pays 50% until each have paid $10,000. All other procedures related to IVF such as lab work, other tests, etc. will be billed and paid subject to deductible and coinsurance. All plans also include medically necessary gender dysphoria treatment subject to accepted medical clinical guidelines and Medical Mutual medical policies
MedImpact, the university’s pharmacy benefit manager for the PPO and High Deductible health plans includes in its standard formulary listing multiple covered contraceptive options selected by physician and pharmacist subject matter experts who collaboratively support MedImpacts Pharmacy and Therapeutics (P&T) Committee. The plan will cover drugs listed in the formulary as long as the drug is indicated for the clinical condition, is prescribed in the appropriate manner, is filled at a participating network pharmacy, and follows other plan rules. HMO members’ coverage also provides benefits for Food and Drug Administration-approved contraceptive methods including, but not limited to, barrier methods, hormonal methods, and implanted devices. Exclusions to available contraceptives include, but are not limited to, male contraceptives and over-the-counter birth control without a prescription.
