Reminder for supervisors

This is the quarterly notice to remind all supervisors (including faculty who supervise staff) that a completed Personnel Action Form (PAF) must be promptly processed whenever an employee is leaving the university for any reason. Please always download a fresh Personnel Action Form from the Employment section of the HR forms page. This newest Personnel Action Form includes a space to indicate the supervisor's name or a designee who will ensure completion of the Termination Checklist and that all applicable items are completed and/or returned by the employee.

Whenever a staff member, faculty member, research associate, senior research associate or postdoc leaves the university for any reason, the PAF must be completed and submitted consistent with the University Termination of Employment Policy and Procedure.  It is the responsibility of the supervisor to ensure this procedure is followed. The PAF should be submitted as soon as the departure is known, but no later than the individual’s departure date.

Failure to process the Personnel Action Form in a timely manner can result in salary overpayment, which may or may not be recovered. Overpayments that cannot be recovered adversely impact operating results. Compliance risk is introduced if an attempt is made to inappropriately charge an overpayment to a grant. Other risks are introduced because the employee continues to have access to information systems and physical facilities. It is important that all staff-related processes and performance reviews be managed in a quality and timely manner.
